Transitioning Between Songs

Is there an easy way to transition using chords, between songs going from one key to another?

Hi, Barbara--

The easiest way is to determine the V chord for the new key and somehow arrive at it from the old key.

For example, if you want to change from the key of C to the key of D, use Am from the key of C and then go to A major and then to A7, which leads to the Key of D.
